Vision and hearing screenings are provided annually at grade levels designated by the Illinois Department of Public Health.  All students in special education programs, students transferring into the district who have not been previously screened, and students referred by a teacher are screened during this annual program.

A parent or guardian will be notified if a student needs further professional evaluation. Please request that your physician complete an examination report.  Return the form(s) to HCHS Health Services.

Please note: The vision screening test is only a screening procedure and is not a substitute for a complete eye and vision evaluation.  If any child shows signs of visual difficulties, he should be evaluated by an eye doctor, regardless of the results of a screening.
